What information do I need to provide in the Health Risk Assessment?

A number of questions are asked to determine lifestyle factors and other health related information. Some information you may want to collect prior to starting your Health Risk Assessment is a list of your medications with the dosages, your height, and weight.

You will be also be asked to provide your waist measurement and your current blood pressure reading, however our Health Ambassador from Dynacare Laboratory will collect that information during your private blood collection appointment.

When convenient, you can return to the place you left off by signing back into your account and clicking on the Get Started button. However, once you’ve completed the Health Risk Assessment your answers will be locked in and you will no longer be able to make any changes.
