Your Personal Information

Will any personal information I provide be shared with 3rd parties?

My Health Report will not sell, trade or exchange any Personal Information that we collect. We will use both the Personal Information and the Personal Health Information that we collect to help us provide the products and services you have requested and to maintain and service your accounts. Our protection of your Personal Information extends beyond the period of your customer relationship with us. If your customer relationship with us ends, we will not disclose your Personal Information to non-affiliated third parties, other than as required by law. Purchasing & Payment

Will any additional tax, shipping or handling charges be added to my order at check out?

Upon completion of your order, an additional HST charge of 13% will be added to your purchase. There are no additional shipping or handling charges.

What forms of payment do you accept?

My Health Report accepts all major credit cards -American Express, Visa and Mastercard.

Our bundled health reports are a way of making preventative health risk information affordable for everyone. Simply choose a health report with the screenings that interest you at an already discounted rate. Take our Free Assessment to help you choose.

Are any of the tests covered by OHIP?

No, our health reports are not covered by OHIP

Will my private insurance carrier cover my tests?

You will need to contact your private insurance carrier to determine if the cost of your health report can be partially or fully reimbursed.

Our Process

How long will it take to complete the process?

The length of time your health report takes to complete will depend on your schedule. It can be as quick as 6 - 10 business days. It all depends on how quickly you can complete the necessary steps.

You can fill out the Health Risk Assessment online as soon as you have registered for your health report. Once completed, you can arrange for our partner to come to your home or office for your blood collection and biometric reading appointment. From the day of your appointment, it only takes 5 - 10 business days to process your blood work and analyze all of your screening data to create your personalized health report.

How can I make the process as quick as possible?

By completing your Health Risk Assessment right away and promptly arranging your blood collection and biometric reading appointment, we can provide you with quick and efficient service. Once we have all of your screening data, it only takes 5 - 10 business days to process your blood work and create your health report.

How long does the blood collection and biometric reading appointment take?

We only need 20 minutes of your time, and we come to you! You can arrange for us to come to your home or even to your office. All we need to do is take a blood sample, take your blood pressure, and measure the circumference of your abdomen.

How long do I have to wait for my health report results?

You will have your health report results within 5 - 10 business days from the date of your blood collection and biometric readings.

Health Risk Assessment

What is a Health Risk Assessment?

Our Health Risk Assessment is an online physical consisting of a health questionnaire. The information you provide allows us to accurately analyze your health risks when we are reviewing your blood work and biometrics. It is important that you answer all questions honestly so that we can provide you with the most accurate assessment of your state of wellness and identify your health risks.

What information do I need to provide in the Health Risk Assessment?

A number of questions are asked to determine lifestyle factors and other health related information. Some information you may want to collect prior to starting your Health Risk Assessment is a list of your medications with the dosages, your height, and weight.

You will be also be asked to provide your waist measurement and your current blood pressure reading, however our Health Ambassador from Dynacare Laboratory will collect that information during your private blood collection appointment.

Can I start the Health Risk Assessment now and then finish it later?

Absolutely! If at any time you need to stop, the information you’ve completed will be saved. When convenient, you can return to the place you left off by signing back into your account and clicking on the Get Started button. However, once you’ve completed the Health Risk Assessment your answers will be locked in and you will no longer be able to make any changes. And remember, you need to finish your Health Risk Assessment before you can book your blood collection and biometric readings appointment.

Why do I need to finish the Health Risk Assessment before my private appointment?

Once you finish your Health Risk Assessment, we can generate and send out your lab requisition form to your Health Ambassador from Dynacare Laboratory. Without this form our Health Ambassador from Dynacare Laboratory will not be able to collect your blood and take your biometric readings. But don’t worry, we take care of this crucial step for you! All you have to do is complete your Health Risk Assessment form and then book your at home or office appointment.

Blood Collection & Appointment Information

Who will do my blood work?

Your appointment will be completed and your blood work processed by one of the leading laboratories in Canada, Dynacare Laboratory.

When can I book my appointment?

You must complete your Health Risk Assessment before you can book your appointment. You can start your Health Risk Assessment as soon as you have registered with My Health Report. Once completed, My Health Report will contact you to schedule an appointment time convenient to your schedule.

Once booked, you will receive a confirmation email with the date and time of your appointment. If you need to cancel or rebook a scheduled appointment, Dynacare Laboratory requires a minimum of 48 hours notice. Appointments that are missed or rebooked with less than the required 48 hours will be subject to an additional fee of $100.00.

Can I change or rebook my blood collection appointment?

If you need to cancel or rebook a scheduled appointment, a minimum of 2 business days notice is required. Appointments that are missed or rebooked with less than the required time will be subject to an additional fee of $100.00. This fee must be paid prior to rebooking your blood collection appointment.

Health Report Results

Are the test results accurate?

The Dynacare laboratory personnel diligently monitor the accuracy and precision of each test. Although each test has its own performance measures and appropriate uses, laboratory tests are designed to be as precise, accurate, specific, and as sensitive as possible. Integral to all Dynacare operations is their extensive Quality Management System, which ensures they meet all stringent regulatory requirements. If you have fasted for the appropriate amount of time before your blood collection, the tests should be accurate.

Are my test results confidential? who will see my results?

All tests results are highly confidential. My Health Report takes all reasonable precautions to safeguard your Personal Information, whether the information is kept electronically or otherwise. This involves ensuring that the only people who view your Personal Information are those who have a need to do so, and only where necessary and appropriate. How long will it take to receive my results?

Your health report will be ready within 5 - 10 business days from the date of your blood collection appointment.

How will I know when my results are ready?

Once your lab work has been processed and your health report has been created, we will send you an email to notify you that your health report is ready. You will be able to access, download and print your health report from your My Health Report online account.

What do I do if I receive an abnormal test result?

If your health report shows that you have results outside what is considered to be the normal range, you can share your lab results with your current health care provider for guidance. Our Wellness Network is also available to you. We can connect you to our network of healthcare professionals to help you get informed consultations and further investigate your results.

Can I have my test results sent to my doctor?

All of your lab reports will be available in your personal account. There you can print your reports and provide them to the healthcare professionals of your choosing.
